Criteria for changing Major

Request Form for Major Change

Student who wish to change their major in academic year 2022/2, Please fill out the request form in the first section and ask for advisor's comment and signature in the second section with transcript and a copy of student ID card.

Please submit the following documents within December 28, 2022 before 5 pm via this link
